Please find below the waiver that you agree to when registering with Embodied Pilates:
I acknowledge that I am participating in Pilates sessions offered by Embodied Pilates where I will receive information and instruction about Pilates, physical exercise and health. I am fully aware that the sessions/program/exercises requires physical exertion which may cause physical injury, I am fully aware of the risks and hazards involved.
It is recommended that you consult your GP/physician/medical practitioner before beginning this or any other exercise program. If you have a medical concern/problem or health-related question, please consult with your GP before participating.
These sessions are not a substitute for medical counselling or treatment. If you have any doubts about the suitability of the sessions/program/exercises, you should refer back to your GP/physician/medical practitioner.
In consideration of Embodied Pilates agreement to instruct, assist, and train me, I do here release and discharge and hereby hold harmless Embodied Pilates from any and all claims, demands, damages, rights or causes of action, present and future, arising out of or connected with my participation in this or any programme including any injuries resulting from them. This waiver and release from liability includes, without limitation, injuries which may occur as a result of my participation in any activity, session programme, personal training or instruction.
With this knowledge, I confirm that I am making a voluntary choice to participate and engage in the sessions/program/exercises and accept all and any risk associated with my decision to participate.
To my best knowledge I am in good physical condition and have no disease, physical limitation, health concern or injury that would be aggravated or would be the cause of any injury sustained, before, during or as a result of my participating in activities related either directly and/ or indirectly to Embodied Pilates.
By taking sessions with Embodied Pilates either virtually, in person or via their content online, you understand and agree that Embodied Pilates has no liability to you for any injury or loss you may suffer in connection with any Pilates session. The use of any information provided by Embodied Pilates is solely at your own risk.
I confirm that I have read and understood the above advice and that the information. I voluntarily agree to the above liability waiver and terms and conditions under my own free will.
Cancellation Policy:
24 hours notice is required to cancel or change a session, if you cancel within 24 hours of your session you will be charged in full. If you need to cancel your session within 24 hours prior to your session starting please email hello@embodiedpilates.co.uk
Payment must be made in full prior to the session.
Sessions are not transferable and non refundable.
I have read the above statement and understand the above conditions. I am aware that if I do not cancel my pilates session within 24 hours I will be responsible for payment of the session in full.
Covid -19 Policy:
Please do not attend if you have tested positive for Covid-19.
If you are feeling unwell and/or showing any symptoms of Covid-19, please contact us as soon as possible to cancel your sessions.
If you have to cancel your session due to covid-19 within 24 hours prior to your session, we ask that you provide us with confirmation of a positive lateral flow test.
In this instance we would reschedule your session for a time after your period of self isolation has ended.
If you are unable to provide this information you will be charged and your session counted as normal.
If you give notice before the cancellation period of 24 hours, the session can be rearranged to another time to suit you and Embodied Pilates.
